Treatment for Snoring and Sleep Apnoea

Once a decision is taken to proceed with surgery, a detailed evaluation is carried out. This includes Polysomnography (PSG / Sleep Study) and a procedure called D.I.S.E.

A sleep study is carried out overnight, either in the hospital or at the patient’s home. Most persons opt for the study to be carried out at home as this is most comfortable and costs less. The sensors are applied at night and removed in the morning. A report is generated within a day, which indicates the severity of Sleep Apnoea. It tells the surgeon the degree of obstruction and the harmful effect of this obstruction on the heart rate, oxygen concentration in the blood, and many other parameters. It also points to any other causes of sleep apnoea, such as Central cause, which means due to abnormal control of sleep from the brain. A sleep study is an excellent method to document the Sleep Apnoea, and this study is then repeated a few months after surgery, to document the improvement.

D.I.S.E. or Drug-Induced Sleep Endoscopy is carried out in the operation theatre under supervision of the anesthetist. The person is helped to sleep by the use of a mild sedative. During this short period of sleep, a thin and flexible endoscope is introduced thru the nose and into the throat, from where the palatal and tongue movements can be recorded. This indicates to the surgeon about the level at which the obstruction is taking place, so that he can plan the surgery.

Surgery is usually a multi-level one, meaning many different areas such as the nose/sinuses/palate/base of tongue/tonsils/adenoids etc are dealt with during the surgery.

The results of the surgery are generally good, and some patients are then able to sleep efficiently and comfortably with diminished snoring, and some are even able to sleep without the use of the CPAP machine.
